A new peer-reviewed opinion article in Archives of Reproductive Medicine argues that topical corticosteroids alone are not enough for many patients, calling for an individualized approach combining hormonal, immune, and regenerative therapies.

The article, "A Comprehensive and Holistic Approach to Vulvar Lichen Sclerosus: Looking Beyond Topical Corticosteroids," challenges the conventional single-therapy model for one of the most under-recognized chronic conditions affecting women's genital health.
A Condition Often Misdiagnosed
Vulvar lichen sclerosus (LS) affects up to 1 in 70 women, yet is frequently mistaken for yeast infections or dermatitis, leading to delays that can cause irreversible scarring, sexual dysfunction, and increased cancer risk.

Rethinking Standard Care
While topical corticosteroids remain first-line treatment, Dr. Caplan notes many women continue to experience persistent symptoms or lifelong steroid dependence despite guideline-directed care. She argues LS should be treated as an immune-mediated disease, noting that roughly one-third to one-half of affected women show autoimmune serological abnormalities, with links to thyroid disease, vitiligo, and type 1 diabetes.
