(AHA Services), a for-profit, wholly owned subsidiary of Arkansas Hospital Association (AHA), as its exclusive Chronic Care Management partner. AHA Services partners with organizations to offer a variety of beneficial resources such as education services, financial support services, insurance services, and more to help support the Arkansas Hospital Association's member hospitals.
ChartSpan is a value-based care services organization, offering the largest managed Chronic Care Management (CCM) program in the U.S. The company offers healthcare providers an end-to-end program, allowing them to see increased reimbursements and improved patient outcomes.
