MONSEY, N.Y. - TelAve -- Upstate New York based Horizon Home Care Services announced expansion of their Consumer Driven Personal Assistance Program (CDPAP) coverage areas with a new office in Canton. In addition to this new location in St. Lawrence County, Horizon continues to provide services throughout New York, including Suffolk, Rockland, and Orange Counties; New York City, Rochester, Troy, and Syracuse. CDPAP is a Medicaid program that pays for in-home help provided by a trusted friend or family member..
As a Medicaid-approved and locally-owned Upstate New York home care agency, Horizon is dedicated to ensuring elderly, ill, and disabled New Yorkers receive much-needed in-home care. Their knowledgeable team members guide patients and caregivers step by step, making the qualification and sign up process simple and fast.
